At the Tuyen Quang General Hospital, Dr Vu Thi Kim Chi removed a 3cm long leech from his nose.
The man is now normal.
The doctor said it could have been more dangerous if the leech had moved down into his throat.
It is a common occurrence when people drink from streams when working in the field or going into forests, and this habit should be abandoned, she said.
In March doctors at Cho Don Clinic in Bac Kan Province removed 4 leeches from the nose of a three-year-old child. The family said they often use water from a nearby stream.
